Bridge Clinics

Bridge Clinics provide immediate access to mental health and substance use disorder support services for all adult Delawareans. These clinics are licensed at the ASAM 1.0 Outpatient Treatment Level of Care and offer rapid support for people who need to receive urgent treatment and connect with ongoing care.

What services do Bridge Clinics provide?

  • Screening, treatment, and referral for substance use and mental health disorders.
  • Evaluations by qualified licensed clinicians.
  • Initiation on Medications for Opioid Use Disorder (MOUD).
  • Certified peer specialists who help individuals connect with support services.
  • Transportation to/from facility as needed.
  • Training and distribution of naloxone.
  • Case management.
  • No fee for services, regardless of insurance status.

Bridge Case Management

Nonclinical support services for any individual, regardless of treatment engagement, including:

  • Permanent or temporary housing assistance.
  • Applying for social services.
  • Obtaining identification and documentation.
  • Food and clothing assistance.
  • Transportation.

Bridge Clinic locations

Bridge Clinics are strategically located to provide support to individuals returning to the community from incarceration and their families. Services are available at the following locations:

Mobile Bridge Clinic locations

These are community-based, nonclinical extensions of the Bridge Clinics that bring services directly to individuals in need.

Services include:

  • Engagement and re-engagement with treatment.
  • Access to acute stabilization services (detox, emergency care).
  • Case management.
  • Full staff of certified peer specialists.
  • Partnership with Food Bank of Delaware:
    • Monthly mobile pantry in each county
    • Distribution of bagged lunches, hygiene kits, PPE
  • Community engagement:
    • Attendance at conferences, fairs, and public events
    • Education on:

Encampment Bridge for Unhoused Outreach

This special service provides outreach to unhoused populations, including individuals living in encampments and tent communities across Delaware. Teams of outreach specialists operate in each county in Delaware, with attention given to high-need times of early morning and late afternoon/evening.

Services include:

  • Screening and referral to Bridge Clinics.
  • Access to case management services.
  • On-demand outreach to individuals or locations.
  • Collaboration with a DSAMH wound-care nurse to provide medical care for wounds.
  • Coordination with police agencies on encampment relocation efforts.
